You may contact us via email for non-emergency questions at info@hoffmanobgyn.com. Responses to email messages are typically returned within 48 hours, but please allow adequate time for us to respond to your request before calling to follow-up. We do not check emails on weekends and holidays.
Initial Visits - What to Bring
New patients must bring their insurance card and photo ID with them to the first visit. Without these items the patient will need to be rescheduled.
Missed Appointment Policy
Missed appointments represent a cost to us, to you, and to other patients who could have been seen in the time set aside for you. We require 24 hour notice if you are unable to keep your scheduled appointment. If notice is not received, there may be a $20 charge for missed appointments.
